Hilot is the general Filipino term for a body of manual practices - massage, manipulation, bone-setting and traditional midwifery - carried out by a manghihilot, a practitioner whose knowledge usually passes within a family rather than through formal schooling. It draws on ideas about balance, heat and cold and the movement of air in the body that predate Spanish contact, and it has always operated alongside rather than inside the formal health system: in many rural areas the manghihilot is the first person consulted for sprains, strains and aches. Since the 1990s the state has moved to recognise and regulate parts of the field, with the Philippine Institute of Traditional and Alternative Health Care established by law and TESDA now certifying practitioners for the spa industry, so the version most visitors meet is a licensed therapist in a wellness centre rather than a village healer.
